I think I may have to have an apico surgery....What causes the infection of a root canal?....Is my dentist to blame?

Apicoectomies are performed when there is an infection around the tip of the tooth's root that hasn't healed after performing a root canal. There are several reasons for these to occur, including trauma, continued infection from the tooth and the body's inability to heal the lesion. Most cases are the latter, where the infection existed prior to the root canal and because of the limited blood supply around the tooth, the body is just not able to heal by itself.
