How to set up your Electronic Funds Transfer (EFT) with DentaQuest

If you are an existing Health First Colorado Dental Provider, you can submit your Direct Deposit/ EFT information electronically or via paper submission.

To submit electronically:

1.       Have an electronic file of a voided check with your preferred banking information ready (you can get this by scanning a voided check or asking your bank if they can provide you with one)
2.       Go to http://www.dentaquest.com/dentists/self-registration-page/
3.       Follow the instructions to Create a Health First Colorado Dental Program Provider User Account
4.       Once your account is created, go to www.DentaQuest.com/Colorado, and click on the Login link under “Dentists Page”
5.       Follow the instructions under Plan Messages to submit your EFT information
 
PLEASE NOTE: If you are an Enrolled Health First Colorado Provider, and are unable to create a login or receive a message similar to the below, please call 855-225-1731 for assistance.


To submit via email or fax:
1.       Have a voided check with your preferred banking information ready
2.       Download and print the EFT Form
3.       Fill out the paper application appropriately
4.       To email, scan or upload your EFT form and voided check and email via secure mail to: Participate@dentaquest.com; Please put “CO EFT” in the Subject Line
5.       To fax, send in your EFT form and voided check to 262-387-3927. Please put “CO EFT” in the Subject Line
