DentaQuest Dental Clinical Criteria & Guidelines

Inclusion of referenced materials does not imply coverage or reimbursement. Coverage determinations are governed by applicable plan documents, contracts, policies, and medical necessity criteria. DentaQuest reserves the right to modify clinical criteria and coverage guidelines in accordance with contractual, regulatory, and policy requirements.
- EPSDT requires comprehensive dental services for beneficiaries under age 21, including relief of pain and infection, restoration of teeth, and maintenance of dental health. States must cover medically necessary services even if not listed in the state plan. Adult dental coverage varies by state.
- Association guidelines are evidence sources; final coverage decisions are determined by applicable policy, DentaQuest clinical criteria, and medical necessity documentation.
