If you or your child is a DentaQuest CHIP or Medicaid Member and received care on or after July 1, 2026, you may qualify for one of the following:
Your dental cleaning must be performed by a provider within the Dental Wellness Plan or Health & Well Kids in Iowa Plan network.
For ages 0 months - 2 years
Kit includes a durable zippered backpack, infant toothbrush, toothpaste, brushing chart and stickers.- Also, adult oral health kit is included for the Member’s parent/guardian with educational materials, spinning toothbrush, and a timer to encourage 2-minute brushing and floss.
- For Members ages 0 months to 2 years old who get a preventive dental service within the first 90 days of enrollment and return the Value-Added Services form to DentaQuest.
- Limit of one kit per Member per lifetime
For ages 3 - 8
- Kit includes a durable zippered backpack, toothbrush, toothpaste, brushing chart and stickers.
- Plus, adult oral health kit is included for the Member’s parent/guardian with educational materials, spinning toothbrush and a timer to encourage 2-minute brushing and floss.
- For Members ages 3 years old to 8 years old who get a preventive dental service within the first 90 days of enrollment and return the Value-Added Services form to DentaQuest.
- Limit of one kit per Member per lifetime.
For ages 9 and up

- For Members ages 9 years or older who get a preventive dental service within the first 90 days of enrollment and return the Value-Added Services form to DentaQuest.
- Limit of one kit per Member per lifetime.
Dental Wellness Plan Members may be eligible for a $20 Amazon gift card. Members must complete both activities to be eligible:
- Go to your Dental Wellness Plan dentist for a cleaning.
- Answer all questions on the Oral Health Self-Assessment.
To be eligible:
- You must have a dental cleaning and answer all questions on the Oral Health Self‑Assessment between July 1, 2026, and June 30, 2027.
- You must be at least 19 years old and continuously enrolled in the Dental Wellness Plan with DentaQuest during July 1, 2026 and June 30, 2027.
- Your dental cleaning must be performed by a provider within the Dental Wellness Plan network.
- Your dentist is required to submit the claim to DentaQuest within 180 days of the date of service.
- You must complete the full PreViser Oral Health Survey with DentaQuest, including providing a valid phone number and email address.
- Participation is limited to one gift card per member over their lifetime
Claim your gift
You are required to complete a form online attesting that you’ve met the criteria above and are eligible for the gift card. A DentaQuest representative will review the information to confirm whether or not you are eligible to receive the gift card.
*The gift card can be used to purchase:
- Oral health supplies such as toothbrushes, toothpaste, floss and mouthwash.
- Baby care items such as diapers, bottles and formula.
- School supplies and clothes.
**Member must return the Value-Added Services form to DentaQuest within 60 days of the date of service. Treatments must be provided by an in-network provider while the Member is enrolled with DentaQuest.
