Attention IL Medicaid Providers: Providers are required to revalidate with IMPACT every 5 years. HFS will be sending notices via email to all providers 90 calendar days prior to their IL Medicaid Enrollment end date and a second notice will be sent via email 30 calendar days prior to their revalidation end date. Please be on the lookout for these notices and follow all instructions. Please refer to IMPACT's website for Step-by-step instructions and FAQ or call the Provider Enrollment Center at 877-782-5565. Failure to revalidate will result in termination.
