  • Register for the DentaQuest member portal.
  • Receive sealants from your dentist. (For members 6-14)
  • Fill out the Florida Healthy Behaviors Rewards form and send it to DentaQuest.

Limitations or Restrictions

  • Eligible members have DentaQuest as their Florida Healthy Kids dental plan administrator.
  • Members who qualify will be eligible for the gift card once per lifetime.
  • Florida Healthy Kids (CHIP) members ages 6-14 must meet the qualifying dental service - receive sealants.
