- Oral exams (one every six months)
- Dental x-rays (once per 36 months per patient)
- Teeth cleaning (one every six months)
- Fluoride (two applications per 12 months per patient)
Treatment
- Fillings (once per 12 months per patient per tooth per surface)
- Crowns (one per tooth every 60 months)
- Root Canal (one per tooth per lifetime)
- Extractions (teeth removal) (one per lifetime per patient per tooth)
- Periodontics (one per 36 months per patient per quadrant)
- Dentures (one per lifetime per patient)
