Louisiana Medicaid

DentaQuest provides dental coverage to eligible members of the following Louisiana Medicaid and CHIP plans:

• EPSDT Dental Plan - Medicaid members ages 20 and younger
• Adult Waiver Plan - Medicaid members ages 21 and older who qualify
• Adult Denture Plan - Medicaid members ages 21 and older who qualify

All plans except the Adult Denture Plan

Prevention

• Dental exam (every six months)
• Teeth cleaning (every six months)
• X-rays
• Fluoride treatments
• Sealants

Treatments

• Fillings
• Crowns
• Root canals
• Extractions (teeth removal)
• Other treatments that are medically necessary

Additional

• Sealants (not available on all plans)
• Silver diamine fluoride (SDF) application - a safe, painless, and effective liquid fluoride treatment to slow tooth decay (age requirements/not available on all plans)

Adult Denture Plan

• Complete or partial dentures and associated exams and x-rays
• Nontraumatic dental care received in the hospital
• Exams and X-rays not covered by your health plan
• Up to $200 a year for simple extractions
• Extra preventive benefits (free cleaning and dental exam after visiting a dentist within 7 days of going to the hospital for non- traumatic dental care)
