Most providers can join our network in two ways:

Complete and submit a DentaQuest credentialing application online.
Sign up with The Council for Affordable Quality Healthcare (CAQH) and maintain your credentialing information in one place. There is no cost and online registration is fast and easy.

Please note: If you are a participating provider in AZ, DC, LA, OH, TN or TX, a CAQH application is required. The DentaQuest online application is not accepted for these states.

We encourage you to complete your credentialing application online. However, if you prefer to use a paper application to enroll by mail, please call a DentaQuest provider relations representative.
