How to Access Your Member Portal
1. Identify your plan type:

- Individual Plan (HealthCare.gov)
3. Click on create an account and enter the following information to register:
4. Sign in to your account
- Subscriber/Member ID number
- Your email address
- First and last name
- Your date of birth

I don’t have an account. How do I set one up?
It’s easy to set up. Registering takes only a few minutes from your smartphone, tablet or computer. Once you are set up, you can use the portal as often as you like.
What you need to Register:
1. Subscriber/Member ID number
2. Your email address
3. First and last name
Where can I find my subscriber/member ID number?
Your Subscriber/Member ID number appears on your Member ID card and on the Welcome letter you received from DentaQuest in the mail. If you cannot find your ID number or card, please call the Customer Service number for your state.
Why did I get an error message when I attempted to create an account?
The information you provided to create your account must match the information used when you signed up for your plan. For example, the first and last names must match what is on the dental plan. Contact Customer Service if you need to make any changes.
I forgot my password. Can I create a new one?
Select ‘Forgot Password’ on your state plan login page, then enter the email address you used to set up your account in the ‘username’ or ‘user ID’ field. You will be asked to create a new password.
