Molina Healthcare of New York Child Health Plus 

DentaQuest provides dental coverage to eligible members of Molina Healthcare of New York Child Health Plus. Molina Healthcare of New York Child Health Plus is a free or low-cost health insurance plan for children who are under the age of 19, are not eligible for Medicaid and who have limited or no health insurance.

Prevention

• Oral Exam – Once every six months
• Bitewing x-rays – Three per 12 months
• Teeth Cleaning – Once every six months
• Fluoride – Once every six months per patient

Treatments

• Fillings – One every 24 months per patient per surface
• Crowns – Once every 60 months per patient per tooth
• Root canal – Once per tooth per lifetime
• Extractions (teeth removal) – One per tooth per lifetime
• Periodontics – Once every 24 months, same quadrant 
• Dentures – One every 96 months per patient

Other Services

• Orthodontia (braces) – Once per lifetime
