Dentaquest provides dental coverage to Medicaid-eligible members of Molina Healthcare of New York who are enrolled in the following plans: New York Molina Adult Medicaid, New York Molina Child Medicaid, and New York Molina HARP.

Prevention

• Oral exams (every six months)
• Dental cleanings (every six months)
• Bitewing x-rays (three every 12 months)

Fluoride (every six months)

• Treatment
• Fillings – (one per 24 months)
• Crowns - (one per 60 months)
• Dentures – (one per 96 months)
• Extractions (tooth removal) (one per tooth per lifetime)
• Root canals (one per tooth per lifetime
• Implants – one per tooth, per lifetime

• Orthodontia (braces) – one per lifetime, age restrictions apply
