It's time to get recredentialed!

Thank you for participating in the DentaQuest network. In accordance with the National Committee for Quality Assurance (NCQA) guidelines, providers are required to undergo recredentialing every three years.

If you are a participating provider in AZ, DC, LA, OH, TN or TX, a CAQH application is required. The DentaQuest online application is not accepted for these states.

It is accepted in all states, but AZ, DC, LA, OH, TN or TX require providers to utilize the CAQH application.  If a non-CAQH application is submitted, a credentialing specialist will reach out to the provider to obtain a CAQH application.

One (1) attachment can be uploaded under each of the following application areas: licenses, malpractice insurance, work history, hospital affiliations and office locations.

Yes, we require a copy of the certificate of insurance for each application.

A DEA is required in each state a provider practices in; if you do not have a DEA in a state you practice in, please fill out the DEA Release by clicking “No” under the DEA license field and populating the required fields.

A CDS is required in the following states AL, CT, DC, DE, HI, ID, IL, IN, IA, LA, MD, MA, MI, MO, NV, NJ, NM, OK, RI, SC, SD, UT, WY; if you do not have a CDS license, please fill out the CDS Release by clicking “No” under the CDS license field and populating the required fields. If a CDS is not required in the state that you are applying for, then click N/A.

CAQH Sign Up

The Council for Affordable Quality Healthcare (CAQH) offers a single credentialing application and an online database that contains the information necessary for insurance companies to credential a provider. This allows you to submit and maintain your credentialing information at one location rather than filing with many organizations. There is no cost to file an application with CAQH and it can be completed online. Click here to visit the CAQH website.
