DentaQuest provides dental coverage to Medicaid-eligible members of Extended MLTC, a New York State Department of Health approved Managed Long Term Care Plan.

 

Member Benefits

Prevention

  • Oral exams (every six months)
  • Dental cleanings (every six months)
  • Bitewing x-rays (three every 12 months)
  • Fluoride (every six months)

Treatments

  • Fillings – (one per 24 months)
  • Crowns - (one per 60 months)
  • Dentures – (one per 96 months)
  • Extractions (tooth removal) (one per tooth per lifetime)
  • Root canals (one per tooth per lifetime)
  • Root canal re-treats--once per lifetime, per tooth
  • Gingivectomy--once per 12 months

  • Emergency dental care
