As an out-patient based service provider, Bay Area Community Health offers medical, dental, vision and behavioral health care. The organization focuses on those who are in need based on their income or inability to pay, but anyone can receive care.

Bay Area Community Health uses a holistic approach, providing health education and preventive care, with a focus on social determinants of health.

The Health Access Hero Awards grant will support a program bringing connected care for diabetes and oral health to Bay Area Community Health’s Southern clinic, which serves a high population of agricultural workers. Seasonal workers have inconsistent work and lifestyles, which can contribute to poor health and lack of access to care. Bay Area Community Health seeks to address many of these factors by providing direct support in overcoming social determinants of health and closing gaps in care.

Typically when you see a medical doctor, they don’t do a dental exam. And while you’re crying or having a panic attack, they simply hand you a tissue, they don’t refer you to a behavioral health provider. These are where we see gaps in care, and we treat patients holistically to ensure every part of their health needs are being considered when they see us.
