Bringing prediabetes screenings to underserved communities

Based in the Greater New Haven area, Monitor My Health aims to help people address type 2 diabetes and related conditions such as obesity, heart attack and stroke. Through inclusive policy and focus on social determinants of health, the organization works with individuals to help them understand and manage their health. As a provider affiliated with the CDC, Monitor My Health connects people with the agency’s Diabetes Prevention Program (DPP), a program used by many health and wellness organizations throughout the country.

Monitor My Health’s new MyHEALTHExpress mobile lab will provide free blood sugar and blood pressure screenings, health education, and referrals to the DPP in high-risk areas. The project will target the estimated 79,000 people with undiagnosed diabetes and 944,000 adults with prediabetes in the state. Over one year, myHEALTH Express plans to screen 1,500 residents, identify 600 individuals with prediabetes, enroll 300 in DPP, and conduct Social Determinants of Health screenings for 1,000 individuals. This community-based approach is expected to improve early disease detection, empower health management, and potentially save over $1.2 million in healthcare costs. The Health Access Hero Awards grant will be used to support the operation of the mobile clinic, test kit supplies, and salary support for Community Health Workers.

Most people who have prediabetes do not even know it. And people are not going out just to get tested for prediabetes. If detected early, type 2 diabetes is prevention, even without medication. We're very proud to be paving the way in Connecticut, bringing care, connection and compassion right to people’s doorsteps, so they can take steps to address diabetes, if they have it.
